Own Occupation
The definition of disability is based on your inability to perform the substantial and material duties of Your Occupation. You may also receive benefits under the Residual Disability provision if you restricted to part-time work.
Guaranteed Rates and Benefits
Premiums are guaranteed to remain level during the policy term if you make no changes to the policy. The features and benefits of the policy cannot be changed by the insurance company.

Can I enroll in the future?
No, this is a one-time offering to take full advantage of the plan. Enrollment at a future date is not guaranteed and may require medical approval.
When do I receive benefits?
Monthly Benefit payments begin after 90-days of a qualifying illness or injury and are payable to your age of 67 for a continued disability. If you work beyond age 67, you may continue the policy under the Maximum Benefit provision.
How do I pay the premiums?
The premium is paid through automatic monthly bank draft. For a small fee, policy invoices can be mailed quarterly. You may elect this option when you enroll.
Can I cancel this policy?
