Can I enroll in the future?
No, this is a one-time offering to take full advantage of the plan. Enrollment at a future date is not guaranteed and may require medical approval.
When do I receive benefits?
Monthly Benefit payments begin after 180-days of a qualifying illness or injury and are payable to your age of 67 for a continued disability. If you work beyond age 67, you may continue the policy under the Maximum Benefit provision.
How do I pay the premiums?
The premium is paid through payroll deductions.
Can I cancel this policy?
Yes, the policy can be cancelled at anytime by contacting your employer or the insurance company.
